Payment for professional services is due at the time services are performed unless previous arrangements have been made with our office. For your convenience, we accept Visa, MasterCard, American Express, Discover, Cash and Personal Checks. Should you wish to inquire about third party financing through Wells Fargo, we will be happy to assist you.
If you have dental insurance, we are happy to file your claim as a courtesy. Patients are responsible for any portion not covered by insurance. By supplying us with your most current information, we can expedite your reimbursement from the insurance company. If your insurance information changes, it is your responsibility to notify the office of the change. Please understand: Our financial agreement is with you and not your insurance carrier. We have no control over the coverage you or your employer has chosen, and do not guarantee an exact amount that your insurance carrier will pay. We reserve the right to request, and you agree to pay any claim not paid by your insurance company within 60 days.
Dental benefit plans are designed to share the cost of dental care. These dental benefit plans are not really insurance in the traditional sense but are designed to provide you with assistance in paying for your dental care. A plan may have limitations on the number of office visits, consultations, cleanings, radiographs (X-rays), and various treatments it will cover.

Our administrative staff cannot always answer specific questions about your dental benefits or predict the level of coverage for a particular procedure. Plans written by the same benefits company or offered by the same employer may vary according to the contracts involved. Your plan sponsor (often your employer) is usually in the best position to explain the individual design features of your plan and answer specific questions about coverage.
When you reserve an appointment in our practice, it is considered a confirmed appointment. As a courtesy reminder, we will contact you several days before your appointment. If you need to change your appointment, we request that you call us during regular business hours, at least 2 business days in advance of your appointment. If sufficient notice is not provided (2 business days), we reserve the right to charge a missed appointment fee: $50 for hygiene and $150 for surgical.
