All-on-4® Implants

All-on-4® Implants

All-on-4® dental implants offer a fixed, full-arch tooth replacement option for people who are missing most or all of their teeth or have teeth that can no longer be predictably restored. Instead of replacing each missing tooth with an individual implant, the All-on-4® treatment concept uses four strategically positioned dental implants to support a full arch of replacement teeth.

What Are All-on-4® Dental Implants?

The All-on-4® treatment concept uses four dental implants placed within the upper or lower jaw. Typically, two implants are positioned toward the front of the arch and two posterior implants are placed at an angle to maximize available bone and help avoid certain anatomical structures.

When appropriate implant stability and other clinical requirements are met, a fixed provisional set of teeth may be attached on the day of implant placement. After the implants heal and integrate with the jawbone, the provisional restoration is replaced with the final prosthesis.

Who May Be a Candidate for All-on-4® Implants?

All-on-4® implants may be considered for adults who have extensive tooth loss, severely compromised teeth, or difficulty wearing traditional dentures.

Potential candidates may include people who:

  • Are missing all or most teeth in one or both dental arches

  • Have multiple failing or non-restorable teeth

  • Currently wear removable dentures and want a fixed alternative

  • Have sufficient jawbone to support strategically positioned implants

  • Are in appropriate general and oral health for implant surgery

  • Are committed to good oral hygiene and ongoing dental care

Health conditions, smoking, active gum disease, available bone, and other factors can affect implant healing and long-term success. A comprehensive evaluation is necessary to determine whether All-on-4® treatment is appropriate.

The All-on-4® Implant Process

Treatment begins with a dental examination and detailed imaging to evaluate the teeth, gums, jawbone, bite, and relevant anatomy. A personalized surgical and restorative plan can then be created.

If damaged or failing teeth need to be removed, extractions may be performed as part of treatment. The four implants are then strategically placed in the jaw. Patients who meet the requirements for immediate loading may receive a fixed provisional restoration following implant surgery.

Over the following months, the implants undergo osseointegration, a process in which the surrounding bone heals around and stabilizes them. Once healing is complete, the final customdesigned full-arch restoration can be placed.

Benefits of All-on-4® Dental Implants

All-on-4® implants provide a fixed full-arch restoration supported by dental implants rather than resting directly on the gums. Treatment can provide greater stability for speaking and chewing while restoring the appearance of missing teeth.

Using four implants may also simplify full-arch rehabilitation compared with approaches that require a greater number of implants. For qualifying patients, a fixed provisional restoration may be attached on the day of surgery, which can reduce the amount of time spent without functional replacement teeth.
